Step 1-- Download the Premiere Basketball LEAGUE Form.
Select and click on the format option below which best suits your needs:

Forms Updated: 06-02-09

Step 2 -- Print and then fill out the official form.

Step 3 -- Address the following 5 items:

            1) Select the League Date Your Team Desires
                  2) Provide any schedule request / conflict information
                  3) Provide the appropriate Contact Information
                  4) Provide your Roster Information
                  5) Get Parent Signature *

* NOTE: parent/guardian signature is required.
A final version with signatures may be turned in at or before the first game.

Step 4 -- Include 2 checks for the following amounts
                made out to KC Premiere Basketball:

              1) $50   Forfeit fee...which will be returned after the final game
                         provided your team does NOT forfeit any games.

2) $425 Does NOT Include Practice Times
OR -- $400 Entry Fee Discount for Multiple Teams or Multiple Sessions

Step 5 -- Mail the Registration Form, along with your 2 checks to:
